Scheduling

Select an appointment based on NEW or ESTABLISHED status.

In-person or virtual visits are offered in the state of Washington only.

New Patients

Upon creating your NEW PATIENT appointment, you will receive an invitation to the CharmHealth EHR. 

Please sign up and fill out onboarding documents prior to the first appointment.

Insurance

I do not accept or bill insurance. Appointment fees will be based on the appointment type scheduled. These fees do not cover lab testing.

Payment

Payment is due at the time of service. A receipt of payment or superbill may be submitted to your insurance company for out-of-network benefits. Payment types accepted: credit card, cash, check, and HSA.

All patients are required to be established with a primary care provider (PCP)

Elizabeth Goldberg PhD ARNP does not serve as a primary care provider and does not take after-hours calls. 

If you have an urgent medical concern, please contact your PCP, go to an Urgent Care or Emergency Department, or call 911.

Rates

Discovery Call New patients only (15 minutes) FREE

New Patient visit (60 minutes) $350

New Patient visit (90 minutes) $525

Established Patient visit (30 minutes) $150

Established Patient visit (60 minutes) $300

Established Patient visit (90 minutes) $450

Extended time slots (15 minutes) $75

Cancellation

A credit card on file is required to book an appointment. If you need to cancel your appointment, 48 hours notice is required in order to allow time to fill the empty appointment. If less than 48 hours notice is given, you are liable for a cancellation fee of $200 for new patient appointments or $100 for follow up appointments and your credit card on file will be charged. Exceptions to this policy will be considered on a case-by-case basis.
